Check on a Family Member Service

Sometimes a family member becomes debilitated or homebound. Examples of this include stroke, paralysis, or the development of advanced dementia or parkinsons disease. This becomes a difficult transportation & logistical problem for family members, shuttling their loved one to & from multiple physician offices, hospitals, and therapy facilities. This is especially true for out of town family.

At our practice we make all services, including the physician or specialists come to the home. That way you don't have to miss work and there are no weather issues or need to rely on transportation services. Whether you need consistent home evaluations or occasional check-ins, we provide this service as the ultimate convenience for your already hectic life. Additional charges may apply and is based upon the member's needs.

If your family member is currently not a member, we conveniently initiate their membership at the time of their first visit.
